Home and Community-Based Services

    • Companionship

Monitor the condition of people with disabilities or chronic illnesses and help them with daily living activities, ultimately lowering the anxiety that can come with aging and restoring well-being.

    • Personal Support Workers

Customized support to help clients maintain their independence and dignity. They may provide essential health-related services, such as checking a client's pulse, temperature, and respiration rate. They also may help with simple prescribed exercises and with giving medications. Occasionally, they change bandages or dressings and skin care. Home health aides may also help with medical equipment, such as lifts and ventilators, to help clients breathe.

    • Special Memory and Dementia Care

When someone experiences dementia or memory loss life can quickly become complicated. It is important for those with dementia to receive support from caring individuals who demonstrate empathy and recognise the path of the disease and what they are going through. As well as providing direction and having fun together and offering care and, just like family or a best friend, can make a significant difference. Being there for them when they need someone is essential. Also available for people who have brain injuries, developmental or intellectual delays to help create a behaviour plan and teach self-care skills, such as doing laundry or cooking meals.

    • Homemaker

The base for all homecare services includes light housework and laundry, meal planning for delicious and nutritious meals, and customized support to help clients maintain their independence with dignity.

    • Residential Live-in Care

Live-in care is different from Night Attendant in that the caregiver remains in the patient's home for a 24-hour solution for complex situations, allowing individuals needing continuous assistance to remain in the comfort of home and receive continual comprehensive support without entering a care facility. Terms and conditions apply.

    • Night Attendant

A long awake shift, usually 10-12 hours depending on location, hours may vary depending on the needs of the clients. Includes preparing the care recipient for bed, oral care, toileting, medication reminders, changing clothes and sheets as needed. Caregivers are responsible for being awake and alert if the care recipient wakes up and requires assistance, and to ensure the client does not develop bed sores or fall at night. A cot must be provided, in the same room or nearby, this will allow brief rest periods during short inactive time.

    • Palliative Care

Support for family caregivers and loved ones and liaison with other healthcare providers to ensure comprehensive care and assistance with daily living activities, such as bathing and dressing and symptom management, such as pain, anxiety, and shortness of breath.

Home Health Care Burnaby

The Bridge for Homecare Solutions is a trusted home care agency specialized in assisting individuals and families with the hiring process of both live-in and live-out care for professional, quality Caregivers.

There are two options available to accommodate individual requirements at a more personal level: Permanent Placement, or placement with administration service.

The initial process will help find a quality employee for placement into the home with the right fit:

  • An initial assessment will provide a free consultation to collect information about the Client to make recommendations for a care plan and is the ideal time for the Client to provide a detailed description of the requirements and expectations that are needed for the Client's personalized and individual care plan

  • A detailed care plan including family needs, health and safety risk management, special skills, education and experience required, schedule of work, salary, benefits, starting date and any other requests

  • We begin a search in our database and/or abroad for qualified candidates; each candidate is screened and interviewed to ensure they are a suitable fit with each Client's needs, interviews for the family will be arranged for a selection of candidates

  • The Bridge for Homecare Solutions creates accounts with CRA and Worksafe and permits the Client to operate as an employer and remit a tax-deductible service.

  • After interviews are completed, and a Caregiver who is a good fit has been identified, start date and time are set to begin a three-month trial.

  • The Bridge for Homecare Solutions provides an Agency Agreement and an Employment Agreement for the employee that is compliant to the Employment Standards Act of BC. This must be signed by both parties, before work begins, and at which time, 50 % of the Placement Fee is due.

  • At the end of the 3-month trial, if an amicable agreement has been reached between both parties, the remaining 50% of the Placement Fee is due.

  • Pay rates may vary and determined by the employer, based on the level of care as long as it falls within ESA regulations, prices may be hourly or daily.
